Research Request Form

If you wish to commission research please complete the following Questionnaire. Remember, while it is not necessary to complete the full questionnaire do include as much information as possible. This will enable our researcher to begin a detailed, yet concise, search.

You can submit the form via email, or print and mail to Donegal Ancestry, and upon receipt of the appropriate fee or Credit Card details we will begin processing your application. If you decide to use email to submit your form, please ensure that you are online and that your default email application is open before sending.

(We are planning to upgrade our service to allow you to submit your Credit Card details on-line. In the interim please forward payment details by telephone/fax/mail/post).
